This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure system.
NEW STUDENT ENROLLMENT THIS APPLICATION IS FOR NEW STUDENTS ONLY. IF YOUR CHILD IS CURRENTLY ENROLLED IN ANY OF OUR SCHOOLS PLEASE DO NOT FILL OUT THIS APPLICATION. YOUR CHILD WILL NOT BE REGISTERED THROUGH THIS PROCESS. CURRENT STUDENTS SHOULD BE REGISTERED IN FAMILY ACCESS BY CLICKING THE REGISTRATION 2023-24 BUTTON . It is best to use a laptop, desktop or a Chromebook to complete your enrollment. Tech support cannot be provided if you are using a cell phone to complete the enrollment process. If you have a child currently attending Maize Schools, you will want to exit this application and log into your Family Access account and click on the New Student Enrollment button to complete the preregistration for your new student. IF you are enrolling a student in any of our Pre-Kindergarten or Early Childhood programs, you are in the right place. Please know that they have additional forms at their building that you will need to complete. Call them at 316-722-3105 after you’ve submitted this application. You have the right to refuse to conduct business electronically. If you decline to conduct business electronically, you will be given access to paper versions of the material requiring a signature. Please call our office @ 350-2050 for information about enrolling with paper versions. **Note: Once you submit this form, if you return, you are able to READ/VIEW only.
